First you need to make the Requiem Ruby.

The crafting recipe requires: 4 Redstone Blocks, 2 Diamond Blocks, 2 Emerald Blocks and 1 Nether Star.

And finally, to craft the Requiem Arrow you need:

The crafting recipe requires: 1 Stand Arrow, 1 Requiem Ruby, and Any Tipped Arrow.

Arrow Ruins

Requiem arrows have around an 5-8% chance to be found in any Arrow Ruins main loot chest along with other stand arrows and arrowheads. Furthermore, suspicious stand in an Arrow Ruin has a 33.33% chance of dropping a requiem arrow.
